Physical Therapy

Services are available to assigned active duty service members without a referral from their primary care manager. The Physical Therapy Clinic doesn’t treat neurology, cardiology, women’s health, or complex vestibular cases. All other beneficiaries will be referred to a local network provider for physical therapy services. If a physical therapy referral is generated by a PCM, the Referral Management Office will receive it and schedule an initial appointment if needed. All referrals sent to the network will show up on the Humana Military website. Follow up appointments are scheduled by physical therapy staff. Physical training uniform or equivalent, or appropriate athletic clothing, is required for all appointments. 

Walk-In (self-refer) Hours: Tuesday and Thursday, 7:30 - 8:30 a.m., or until slots are filled. Slot availability varies from week to week.
