Medical Records 

Access Your Own Records on the Patient Portal 

You can access all non-sensitive records through TRICARE Online, the MHS GENESIS Patient Portal, or both. 

If you obtained medical care at an MTF(s) before their transition to the MHS GENESIS Electronic Health Record, these records can be obtained through TRICARE Online until October 2024. The 20th Medical Group transitioned to MHS GENESIS on Sept. 24, 2022. If the care occurred at an MTF using MHS GENESIS, your records are in that patient portal, found under the Health Library tab. 

As a matter of convenience, the login is the same for both sites; the MHS GENESIS Patient Portal can be accessed from TRICARE Online as shown below. TRICARE Online can no longer be used for appointments, prescriptions, or secure messaging; these are now done through the MHS GENESIS Patient Portal. Records Requests 

You may request a copy of your recent medical records by accessing the DD Form 2870, Authorization for Disclosure of Medical or Dental Information. Instructions how to fill out/submit the form via MHS GENESIS are provided at the bottom of this page. 

  1. DD Form 2870 can be submitted through the MHS GENESIS Patient Portal to Shaw AFB Records Release of Information, or in person at the 20th MDG medical records front desk. 
  2. The release of information process may take up to 30 days. You will be called or emailed when medical records are ready to be mailed/picked up. 
  3. *Requests for mail or pickup must be specified in the message*

If you need copies of records from an off-base clinic, you may send us the completed DD Form 2870 to send to the off-base clinic. Instructions how to fill out and submit form are provided at the bottom of the page. 

  1. Ensure off-base clinic’s fax number and mailing address are provided. 
  2. DD Form 2870 may be submitted through MHS GENESIS Patient Portal to Shaw AFB Release of Information, or in person at the medical records front desk. 
  3. *Requests for mail or pickup must be specified in the message* 
  4. If you have an appointment with an off-base provider within one week, medical records must be notified to expedite the request. 

Release of Information for another adult to pick up medical records 

If you are 18 years of age or older and would like another adult to pick up/request your medical records, you may complete a DD Form 2870, Authorization for Disclosure of Medical or Dental Information. 

  1. If your child is under the age of 18, you do NOT need a DD Form 2870. 
  2. When patient is signing for other adult to pick up records, Relationship to Patient (box 12) must say “SELF” 
  3. When other than patient is signing for medical record release (box 11), adult must print name and specify relationship to patient (box 12) 

National Personnel Records Center 

Records retired before 2002 are kept at the NPRC in St. Louis, Missouri. These records are requested by following instructions on the NPRC Website. 

  1. Information on how to request medical health records while you were in the service is found at Veterans' Medical and Health Records | National Archives. If you were in the service and are requesting records with your physician within one or two years, contact medical records for guidance. 
  2. Information on how to request military retirees and dependents is found at https://www.archives.gov/personnel-records-center/other-medical-records. If you were seen at a military treatment facility within the last two years, contact medical records for guidance. 
  3. Information on how to request records during an inpatient stay is found at Clinical (Hospital Inpatient) Records for former Active Duty Personnel | National Archives. If you were inpatient within the last year, contact medical records for guidance. 

PCS, In- and Out-Processing, Separating, and Retiring 

Before PCSing, retiring, or separating, orders must be sent electronically via the MHS GENESIS Patient Portal by choosing the option to “Message my Provider,” then searching “Shaw Records Release of Information.” 

  1. If you are retiring or separating, you are entitled one copy of your medical records. Requests may be given to members via one CD or one paper copy. Duplicate copies of a medical record may be subjected to fees. Contact medical records for more information. 
  2. Once we receive a copy of your active duty PCS orders, we will ship your hardcopy medical record (created before 2015) during the month of your report date. If your gaining base has not received your hardcopy record one month after your report date, contact 20th MDG Medical Records right away. 

Fees 

Fee may be applied to duplicate record requests. Contact medical records for guidance. 

  1. Electronic Records: Flat rate fee of $13.25 
  2. Hardcopy Records: 15 cents per page plus flat rate fee of $13.25 

How to message Medical Records from the MHS GENESIS Patient Portal 

  1. Log on to MHS GENESIS Patient Portal 

  1. Click “Message My Provider” 

 

  1. Click “Send a Message” 

 

  1. Enter the patient for whom the message is sent. In the “To” box, search ‘Shaw Records Release of Information.’ Identify the subject, provide relevant message in the message area, and then click “Choose File” to search for the DD Form 2870 or other applicable documentation.
